Qualified candidate must:
- Be a team player
- Have two years of nursing experience (home health experience is not required); LPN bridge to RN is acceptable
- Demonstrate strong communication & documentation skills
- Demonstrate the initiative & confidence to make independent nursing decisions
- Demonstrate solid clinical skills

Responsibilities
- Assess patient health status in home settings, document findings, and update care plans accordingly.
- Administer medications, manage wounds, and monitor vital signs with attention to safety and regulatory compliance.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient-centered care.
- Provide patient education on disease management, treatment plans, and self-care techniques to promote independence.
- Utilize EMR/EHR systems for accurate charting, case management, and secure information sharing.
- Manage caseload efficiently, prioritize visits, and escalate urgent concerns as needed.
- Coordinate weekly patient visits with the LPN staff.

Pay: $38.00 - $44.00 per hour
