Patient Service Navigator

Title : Patient Service Navigator Lewiston Maine
What You’ll Do :
- Assists in the development, establishment, and evaluation of team documentation systems in conjunction with Supervisor to assure accuracy, timeliness and efficiency.
- Maintains up to date knowledge base on all insurance forms, chart forms, insurance paperwork requirements, and shares information with team routinely.
- Establishes priorities for team paperwork to be done on a daily / weekly / monthly basis.
- Ensures review of all paperwork for completeness of forms on a regular basis.
- Develops and maintains accurate systems to ensure timely coordination of all multi-discipline documentation.
- Manages scheduling of clinical staff visits.
- Ensures appropriate assignment of referrals in consultation with supervisors.
- Assures service satisfaction though appropriate communication with staff and client.
- Data entry into Electronic Health Record (EHR) of some OASIS & staff timesheets
- Maintain Clinical Scheduling program.
- Assign patients to HHA / Collate medical teaching sheets with POC
- Implement and maintain Master schedule.
- Maintain daily / Holiday & weekend schedules.
- Complete to do lists as assigned
