What You’ll Do
As a Hospice House Nurse Practitioner, you’ll provide direct, compassionate care to patients receiving end-of-life services in a residential hospice setting.
You will:
- Perform comprehensive assessments and ongoing clinical management of hospice patients
- Manage pain and complex symptoms using evidence-based palliative care practices
- Collaborate closely with hospice nurses, social workers, chaplains, and physicians
- Lead goals-of-care discussions and support patients and families through end-of-life decision-making
- Provide timely visits and clinical interventions to ensure comfort and dignity
- Document care accurately within the EMR
- Participate in interdisciplinary team meetings and care planning
- Support a calm, supportive environment for patients and their loved ones
What You’ll Bring
- Master’s degree in Nursing and NP certification (AANP, ANCC, or equivalent)
- Active Maine APRN license
- Hospice or palliative care experience strongly preferred
- Comfort managing end-of-life care in a residential or inpatient hospice setting
- Strong clinical judgment and ability to work both independently and collaboratively
- Excellent communication skills with patients, families, and care teams
- Commitment to compassionate, patient-centered care
- ACHPN certification (or willingness to obtain within 2 years) preferred
