What You’ll Do:
- Provide skilled nursing care to patients in their homes in accordance with the plan of care developed by the RN.
- Perform clinical tasks including medication administration, wound care, and monitoring of patient conditions.
- Observe, document, and report changes in patient status to the supervising RN and care team.
- Educate patients and caregivers on disease management, medications, and preventive care to support recovery and independence.
- Promote patient and family engagement in care to improve health outcomes and self-management.
- Assess the home environment for safety and make recommendations to reduce risks and support recovery.
- Collaborate with an interdisciplinary team to support patient goals and continuity of care.
- Participate in health education and preventive care initiatives within the community.
What You’ll Bring:
- Graduate of an accredited practical nursing program; successful completion of an approved pharmacology course.
- Current LPN license in the State of Maine and CPR certification required.
- Minimum of 2–3 years of recent nursing experience in acute care, rehabilitation, or home health setting.
- Strong clinical assessment skills with the ability to work independently in a home setting.
- Experience with wound care, medication management, and chronic disease education preferred.
- Excellent communication and documentation skills, with the ability to collaborate effectively with an interdisciplinary care team.
- Reliable transportation, valid Maine driver’s license, and proof of auto insurance required.
