Location: Eastern MassachusettsSchedule: Full-Time | Primarily Remote with Travel 1-2 Days per WeekCompensation: $115,000 Annual SalaryBenefits: Comprehensive Benefits PackageWe are seeking an experienced MDS Coordinator (RN) to provide regional coverage and remote clinical support across skilled nursing facilities in Eastern Massachusetts. This role supports facilities during MDS vacancies, leaves, and transitional periods.This position is primarily remote with required on-site travel 1-2 days per week.Responsibilities:Complete and submit MDS assessments and resident interviewsDevelop and update care plans; attend care plan and clinical meetingsReview assessments for accuracy and complianceSupport PDPM, CMI, QM, and QRP processesCommunicate effectively with interdisciplinary teamsTravel to assigned facilities as needed (1-2 days/week)QualificationsActive RN license in Massachusetts (or ability to obtain)Minimum 2 years MDS experience requiredSkilled nursing / long-term care experience requiredStrong attention to detail, organization, and communication skillsComputer savvy and comfortable working remotelyTeam-oriented and adaptable to multiple facility environmentsWhat We Offer$115,000 annual salaryFull benefits including health, dental, vision, PTO, and holidaysRemote flexibility with limited in-state travel
