Andover Forest Post Acute Care Center is seeking an MDS Coordinator LPN Licensed Practical Nurse to fill a full-time, salaried position in our skilled nursing facility.
The MDS Coordinator LPN Licensed Practical Nurse duties will be making sure that all paperwork is completed, and the facility is receiving the maximum reimbursements entitled. You will collect data and gather information from staff and resident to create a detailed, accurate and comprehensive clinical assessment.

LPN: $90,000 - $95,000 per year (DOE)
