The Role: Hospice RN Case Manager
As a Hospice RN Case Manager, you’ll be the heart of our care team, visiting patients in their homes or facilities and ensuring their physical, emotional, and spiritual needs are met with dignity and compassion. You will be supported by a team that believes in making space for humanity, comfort, and grace at every step of the journey.
Key Responsibilities:
- Provide holistic nursing care to hospice patients and their families
- Collaborate with the interdisciplinary team to create and implement care plans
- Support caregivers through education, reassurance, and attentive communication
- Maintain accurate and timely electronic documentation
What We’re Looking For
- Licensed RN in the state of Idaho (required)
- Hospice experience preferred—but we’re happy to train the right candidate
- Comfortable with electronic medical records (EMR)
- Compassionate, calm, and grounded in presence
- Excellent communicator and naturally organized
