Looking for someone great to join our expanding team! Most of our treatment coordinators are social workers, but other human services bachelors degrees are considered. 1 year of experience working with those who have substance use disorder is preferred

Anchorage is a residential treatment center for those with substance use disorder and mental health conditions. The Treatment Coordinator collaborates with clients, clinical staff, community-based resources, and others, to provide treatment services according to the treatment plan while considering cultural differences and special needs of clients to help obtain stable, supportive discharge plans including housing, treatment and medical services. Knowledge on Minnesota assistance/resources recommended.

Duties and Responsibilities:

  • Develop and maintain working knowledge of community resources available to clients, including: health insurance options, housing, medication resources and support groups.
  • Participate in client staffing to maintain consistent communication with counseling staff.
  • Meet individually with clients determining needs and facilitating resources; helping clients in their transition from different levels of care
  • Facilitate referrals to substance use disorder and/or mental health services as indicated by a client's medical provider, comprehensive assessment, or treatment plan. Assist with referrals to economic assistance, social services, housing resources, and prenatal care according to the client's needs.
  • Provide life skills advocacy and support accessing treatment follow-up, disease management, and education services, including referral and linkages to long-term services and supports as needed.
  • Assist in coordination with client’s significant other to help in treatment planning whenever possible and meet with families, probation, and social workers to develop transition and contingency plans. Assist in coordination with and follow up for medical services as identified in the treatment plan.
  • Maintain appropriate charting to communicate clients’ transitional plans and documentation of the provision of care coordination services in the clients’ files.
  • Coordinate correspondence with Probation, CPS, and other concerned persons.
  • Transport the client to appointments, stores, court hearings, family visits and transfer locations.
  • Perform other duties as assigned.

Qualifications

1. Must have either:

(i) a bachelor's degree in one of the behavioral sciences or related fields; or

(ii) current certification as an alcohol and drug counselor, level I, by the Upper Midwest Indian Council on Addictive Disorders;

2. A minimum of 2,000 hours of supervised experience working with individuals with substance use disorder.

3. Have successfully completed 30 hours of classroom instruction on care coordination for an individual with substance use disorder is strongly preferred. ( If not we will arrange for this training after hire). This is a state recognized certificate

4. Computer literacy is required; proficiency is desirable but not required.

5. Prior experience with an Electronic Health Records (Procentive) system strongly preferred.

6. Must be able to maintain positive public relations with referring agencies and the business community at large.

7. Must possess good verbal skills and be capable of presenting positive professional image.

8. Must have a working knowledge of issues regarding confidentiality.

9. At the time of hiring, must be free of problematic substance use for at least the two years immediately preceding and sign a statement attesting to that fact.

10. Background check through the Department of Human Services.

This is a full-time, benefits-eligible position working Monday - Friday days. Paid holidays, with no on-call
