Position Summary
As an Outreach Coordinator you will connect with the care managers we already partner with, strengthening those relationships and checking in regularly to identify patients who may benefit from our services. You’ll handle warm outreach, follow-up calls, and friendly, consistent communication to ensure our partners always feel supported and informed. You’ll track referrals, organize updates, assist with light administrative tasks, and make sure all information is accurately logged in our systems. This position is perfect for someone who enjoys customer service, relationship-building, and organized follow-through. Outreach Coordinator Responsibilities
- Serve as the primary point of contact for care managers regarding scheduling, service updates, and ongoing care needs.
- Monitor and track referrals, ensuring a smooth enrollment
- Respond promptly to patient or care manager inquiries, resolving issues with professionalism and care.
- Make calls, moniter your inbox and confirm all inquires are attended to swiftly
Outreach Coordinator Requirements
- Previous experience in a healthcare, homecare, or customer service coordination role preferred.
- Excellent verbal and written communication skills.
- Ability to manage multiple priorities and deadlines.
- Proficiency in Microsoft Office and staffing software (HHA Exchange or similar).
- Strong problem-solving skills and attention to detail.
- Bilingual (English/Spanish) a plus.
