Licensed Practical Nurse (LPN) – Behavioral Health
Shifts: 12-Hour Day & Night Shifts
Location: Hopkinsville, KY

Position Summary
The LPN plays a key role in supporting patients’ mental and physical well-being by providing hands-on care, monitoring conditions, and assisting the healthcare team in delivering effective treatment.
Responsibilities:
Provide direct patient care and monitor behavioral and physical health status
Administer prescribed medications and monitor patient responses or side effects
Accurately document care provided and report changes in patient condition to the RN or physician
Review and transcribe physician orders
Assist with patient assessments and care planning
Work collaboratively with the interdisciplinary healthcare team
Educate patients and families about treatment and medication plans
Qualifications:
Active, unencumbered LPN license (required)
Graduate of an accredited nursing program
Strong communication and clinical documentation skills
Ability to maintain professionalism and clear boundaries
Experience in behavioral health or psychiatric care preferred
