Job Title: Physical Therapist - Home Health
Location: Elizabethtown, PA
Setting: Home Health
Pay Range: $2,391.40 - $2,510.97 / week
Schedule: 8-Hour Days (Note: Additional shifts or rotation not specified)
Contract Length: 13 Weeks
Candidate Type: Local and traveler friendly, Local candidates only – travel stipend not available, Traveler friendly – travel reimbursement available

Requirements:

• Qualified applicants MUST have at least 2 years of experience in Home Health
• Valid Physical Therapist license
• Be willing to obtain Pennsylvania state licensure

Responsibilities:
As a healthcare professional in a home health setting, you will:

• Deliver one-on-one, high-touch care to patients in their homes, providing skilled PT services.
• Conduct assessments and create individualized care plans tailored to each patient’s needs.
• Collaborate with families and interdisciplinary providers to improve patient outcomes.
