Help improve access to affordable medications in your community! Ampla Health is seeking a Pharmacy Outreach & Enrollment Specialist to connect patients with pharmacy programs, remove medication barriers, and support better health outcomes.

Key Responsibilities:

• Conduct outreach to enroll patients in pharmacy programs (340B, assistance programs, refills, delivery, discounts)
• Identify patients facing medication access or cost barriers and help coordinate solutions
• Track enrollments and maintain accurate records in EHR and pharmacy systems
• Serve as liaison between patients, pharmacy team, providers, and community organizations
• Educate patients about pharmacy and Ampla Health support programs (CalAIM, WIC, transportation, insurance, etc.)
• Participate in outreach events, community presentations, and pharmacy initiatives
• Maintain HIPAA compliance and patient confidentiality

What You Bring:

• High school diploma or equivalent
• Minimum 2 years in healthcare, pharmacy, or patient services
• Strong communication and customer service skills
• Knowledge of insurance, prescription benefits, or assistance programs preferred
• Valid CA driver’s license, insurance, and ability to travel
• Computer skills (Microsoft Office); EHR experience a plus
• Bilingual preferred
