What are the responsibilities and job description for the Program Coordinator - Men’s Phase II position at Amos House?
Position Summary: Amos House is excited to continue expanding our programs and offerings to support the needs of our guests. This role plays a critical part in supporting the needs of Amos House guests who are transitioning from the 90-day program to continuing support in phase II in the men's house.
This role offers case management and support to adult individuals who are residents in Amos House's permanent supportive housing.
This role will report to the Clinical Supervisor and is full-time, 40-hours a week, exempt.
Duties and Responsibilities:
- Crisis Intervention, as needed, including referral to detox in the event of re-occurrence.
- * Ongoing orientation and education regarding the rules and expectations of the 90 Day Program
- * Supervision of attendance at AA/NA meetings
- * Case Plans, Progress Notes, Releases of Information and records of employment and medical/mental health information. Program coordinator will report on progress of each guest during weekly supervision meeting with the clinical director. Coordinator will document progress in Clarity as well as AWARDS and will bill for services in AWARDS with back-up case notes that match treatment/case plans.
- * Coordinator will facilitate at least 3 groups per week in the Day Program; including Uplift and other topic discussion meetings.
- * Coordinator will meet 1-1 with each House Manager every week to review their performance in role of House Manager and work on any corrective actions needed in order to ensure the best treatment possible for our guests.
- * Coordinator will meet with the clinical director at least one time per week for 1-1 supervision.
Qualifications:
- Must have 3-5 years’ experience in providing care coordination/case management services.
- * Candidate must demonstrate knowledge and experience with SUD/Behavioral Health Disorders and a deep commitment to working with individuals who are justice involved, formerly unhoused and struggling with addiction.
- * Advanced clinical degree or LCDP a plus and CPRS/CHW certification within one year of hire will be a requirement for ongoing employment.
- * Must have excellent computer skills and become proficient in AWARDS and Clarity for required documentation.
- * Bi-lingual candidates are encouraged to apply.
