Position Summary

The Program Coordinator oversees the daily operations and case management services for Amos House’s Men’s 90-Day Transitional Housing Program. This role provides leadership, structure, and direct support to ensure residents receive the guidance and resources needed to achieve stability, sobriety, and long-term housing.

The Coordinator manages intakes, assessments, and case plans for all residents, supervises House Managers, and coordinates care with community partners such as correctional facilities, treatment centers, and hospitals. This position plays a key role in maintaining a safe, supportive, and recovery-focused environment.

Key ResponsibilitiesProgram Coordination & Supervision

  • Supervise, schedule, and support House Managers, including hiring, training, coaching, and performance management.
  • Lead weekly house meetings with staff and residents to address community living, conflict resolution, chores, and expectations.
  • Ensure consistent program coverage, communication, and adherence to policies and procedures.
  • Serve as on-call support for the Men’s 90-Day Program during evenings, weekends, and holidays.

Case Management & Resident Support

  • Conduct resident intakes, assessments, and individualized case plans.
  • Meet weekly with each resident to review progress, barriers, and next steps toward self-sufficiency.
  • Provide crisis intervention, including referrals to detox, treatment, or emergency services as needed.
  • Facilitate at least three resident groups each week, including Uplift and other recovery or skill-building discussions.
  • Support residents’ participation in recovery meetings, employment, and mental health services.
  • Collaborate with external partners, including ACI, Providence Center, detox facilities, hospitals, and probation/parole officers.

Data, Documentation & Reporting

  • Maintain timely, accurate documentation in Clarity and AWARDS systems, including case notes, progress updates, and income tracking.
  • Complete all Medicaid billing for eligible residents under CPRS/CHW or Home Tenancy services.
  • Ensure all documentation aligns with treatment goals and meets internal and regulatory standards.
  • Provide weekly progress updates and participate in supervision with the Clinical Director.

Program Transition & Outcomes

  • Support residents in preparing for next steps after completing the 90-Day program.
  • Coordinate transfer of cases to the Phase II Case Manager as residents move into permanent housing within Amos House.

Qualifications

Must-Haves

  • Licensed Chemical Dependency Professional (LCDP) or Bachelor’s/Master’s degree in Social Work, Human Services, or a related field with experience supporting individuals with co-occurring disorders.
  • Demonstrated experience working with individuals who are unhoused, formerly incarcerated, or impacted by substance use.
  • Strong crisis intervention, communication, and documentation skills.
  • Computer literacy and experience with data entry or client management systems (e.g., Clarity, AWARDS).
  • Valid driver’s license and reliable transportation.

Nice-to-Haves

  • Certified Peer Recovery Specialist (CPRS) with at least 12 consecutive months of sobriety and relevant experience.
  • Master’s degree (MSW) or clinical licensure (LCSW, LMHC).
  • Experience supervising staff or managing residential programs.
  • Bilingual or multilingual proficiency (Spanish preferred).
  • Familiarity with Medicaid billing processes for CHW/CPRS or tenancy support services.

Pay: $55,000.00 - $65,000.00 per year

License/Certification:

  • LCDP (Preferred)
  • certified peer recovery specialist (CPRS) (Preferred)
