Description

The Hospice Specialist serves as a mission-driven ambassador responsible for developing, strengthening and stewarding relationships across the healthcare continuum to promote access to exceptional hospice and palliative medicine services. This role works collaboratively with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, social workers and other healthcare professionals to increase appropriate referrals, improve patient access and support organizational growth.


Through education, consultation and partnership development, the Hospice Specialist advances the organization's commitment to compassionate, patient-centered care while serving as a trusted resource to the medical community. Success in this role is measured by the cultivation of strong referral partnerships, growth in referral and admission volumes, expansion of market presence and the ability to align business development efforts with the mission and values of hospice and palliative medicine.

Requirements

ESSENTIAL DUTIES & RESPONSIBILITIES:

•Demonstrate knowledge of federal, state and other regulations governing hospice care

•Work with supervisor to design and carry out a strategic growth plan for health care providers

•Make frequent and ongoing contacts with all area hospitals, adult care homes, skilled nursing facilities, home health providers and key physicians based on strategic plan

•Develop, maintain and analyze on an on-going basis data on all providers

•Provide regular and informative reports on all activities and future strategies

•Work closely with care teams to maintain knowledge of special needs or concerns

•Educate providers about hospice eligibility and the benefits that hospice utilization offers to the medical community, patients and families. Educate providers about referral process and assist as needed.

•Coordinate ongoing education for all new and current physicians in the service area making regular visits to key physician offices

•Address referral partner complaints or concerns, collaborate with team and follow-up with customer to ensure satisfaction

•Maintain brochures and appropriate written materials in health care provider locations

•Demonstrate understanding of nursing home regulations

•Work with interdisciplinary team members to establish and maintain positive working relationships with referral partners

•Develop educational materials and programs for facility personnel

•Cultivate relationship with hospitals across geographical service area

•Cultivate relationships with home health and related providers through regular and ongoing contacts

•Identify and coordinate education programs for providers

•Assist director of public relations and community engagement specialist with coordinating and implementing special days such as doctor’s day, nurse’s week, etc.

•Serve as hospice representative on appropriate community initiatives, other duties as assigned


KNOWLEDGE, SKILLS & ABILITIES:

•Demonstrate understanding of nursing home regulations and how to partner with them

•Demonstrate understanding of ways to partner with home health agencies

•Demonstrate understanding of ways to partner with facilities 

•Demonstrate understanding of ways to partner with hospitals

•Demonstrate understanding of ways to partner with any other facility agencies

•Identify and coordinate education for providers and our partners


MINIMUM QUALIFICATIONS:

Education:

•Bachelor’s degree in marketing, human resources, nursing or related field preferred


Experience:

•Minimum 2-3 years professional experience required, healthcare sales, business development, account management or referral development

•At least 2 years experience in end-of-life care preferred

•Hospice experience strongly preferred

•Demonstrated knowledge of hospice and palliative care

•Proficiency in Microsoft office, email, CRM platforms and routine reporting tools

•Ability to work independently, manage a territory prioritize time and follow through on commitments with minimal supervision

•Strong presentation, communication, listening, negotiation and consultative selling skills

•Demonstrate ability to build trusted relationships with physicians, discharge planners, facility leaders, clinicians and community partners


PHYSICAL DEMANDS:

•Visual acuity to read written documentation and computer tasks

•Speech and hearing skills necessary for telephone interaction and face to face contact

•Ability to sit extended periods of time and perform administrative tasks

•Ability to stand, bend and lift an average of 30 pounds

•Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time

•Ability to walk, squat, kneel, climb and balance


SPECIAL REQUIREMENTS:

•Must have an automobile with required insurance coverage and state driver’s license
