Social Worker, LMSW/LCSW
StartDate: 5/11/2026 Pay Rate: $1600.00 - $2400.00
POSITION SUMMARY – LMSW Social Worker
POSITION DUTIES – Assess patient/family problems and resources, taking a leadership role in the development, implementation and outcomes evaluation of a plan for nursing care. Provides professional nursing care and interventions to acutely ill adult patient populations. Provide service to one or more age groups, including young adults through geriatric patients in a manner that demonstrates an understanding of the functional/developmental age of the individual served. Complete SW assessments, SW Mental Health assessments, Title 36 petitions as needed, DC planning as needed, homelessness, substance abuse, SDOH consults, maternal child assessments, DCS and APS reporting as needed.
MINIMUM REQUIRED QUALIFICATIONS – Arizona LMSW or LCSW; Minimum of 2 years of experience and the ability to float to at least 2 levels of care.
LENGTH OF ASSIGNMENT – 13 Weeks
SHIFT / HOURS PER WEEK – Typically 0800-1630, weekends would be maybe every 3-4 weeks if at all
SYSTEMS – Cerner
