POSITION SUMMARY – RN Lead Case Manager
POSITION DUTIES – Assist with discharge planning, utilization review, family conferences, team conferences, and other required patient needs and resources to facilitate a safe discharge.
MINIMUM REQUIRED QUALIFICATIONS – OK/compact RN license; BLS; 2 years recent social services case management experience in an inpatient setting
PREFERRED QUALIFICATIONS – CCM, ACM, ARN; Previous leadership or supervisory experience; Acute/rehabilitation hospital experience
LENGTH OF ASSIGNMENT – 12 weeks
