POSITION DUTIES – Assessment of discharge needs, Social determinants of health, and discharge planning/referral for Acute Medical and Surgical patients.
MINIMUM REQUIRED QUALIFICATIONS –
- OH/compact RN license
- BLS
- 3 years Acute Care Case Management experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Days, M-F, 730a-4p; No call, no holidays, no weekends
SYSTEMS – Epic
