POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization in an Acute Inpatient Rehab Unit
MINIMUM REQUIRED QUALIFICATIONS – AR/compact RN license; BLS; 2 years case management experience in an acute hospital setting
PREFERRED QUALIFICATIONS – Previous experience in clinical liaison, marketing, or healthcare sales.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8a-5p (32hrs/wk guaranteed). Scheduled holiday and weekend coverage.
SYSTEMS – Meditech
