POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization in an acute rehab setting

MINIMUM REQUIRED QUALIFICATIONS – TN/compact RN license; BLS; 2 years recent acute hospital case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:00 AM-5:00 PM with every other weekend

START DATE – ASAP
