POSITION DUTIES – Care management and discharge planning
MINIMUM REQUIRED QUALIFICATIONS – IN/compact RN license; BLS; 4 years recent case management experience in an acute hospital setting; Care management and discharge planning experience in hospital setting
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00am-4:30pm, holidays required that fall during contract
