POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
MINIMUM REQUIRED QUALIFICATIONS – SC/compact RN license; BSN; BLS; 2 years case management experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break
LENGTH OF ASSIGNMENT – 9 weeks
SHIFT / HOURS PER WEEK – 7:00AM - 5:30PM
SYSTEMS – Midas
