POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 2 years case manager experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break; Knowledgeable in Meditech, Midas, and Interqual.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00AM-4:30PM with every other weekend
SYSTEMS – Meditech, Midas, Interqual
START DATE – ASAP
