POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 2 years acute hospital case management experience; HCA & Midas experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00AM - 5:00PM
SYSTEMS – Midas
