POSITION SUMMARY – RN-Case Manager/Utilization Review/CDI
POSITION DUTIES – Minimum Data Set RN in an acute hospital setting; Facilitates patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating, and monitoring resources to achieve optimal clinical outcomes, utilization of services, and financial goals; Oversees the discharge process for all patients within the unit/core service area; Integral to the team's effort to establish a safe and appropriate discharge plan/plan of care; Possess a working knowledge of the spectrum of hospital and post-discharge services that are available.
MINIMUM REQUIRED QUALIFICATIONS – NY RN license; BLS; 1.5 years recent case management experience (within past 2 years) in an acute care facility; MDS experience; EPIC experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 9:00 am - 5:30 pm
SYSTEMS – Epic
