This Home Hospice RN role is based in and around Augusta, Maine, a welcoming capital city known for its historic downtown, riverfront views, and easy access to nature. In this position, you will provide compassionate hospice nursing care to adult and geriatric patients in their homes, assisted living residences, and other community settings. Your focus will be on promoting comfort, dignity, and quality of life for patients with life-limiting illnesses, while supporting families through education, guidance, and emotional care. A typical day includes traveling to scheduled home visits in the Augusta area, performing comprehensive assessments, managing pain and symptoms, administering medications and treatments, and updating individualized care plans. You will communicate closely with physicians and an interdisciplinary team to coordinate care, adjust interventions, and ensure that patient and family goals are honored. Documentation is completed in an electronic medical record system, often during or soon after each visit, using a laptop or mobile device. Responsibilities include: Holistic nursing assessments in the home environment Ongoing management of end-of-life symptoms and comfort measures Medication administration, teaching, and reconciliation Education and support for patients, families, and caregivers regarding hospice philosophy and what to expect at end of life Coordination of services and supplies to support safe, comfortable care at home Participation in case conferences and interdisciplinary care planning Timely documentation and communication with the care team Occasional on-call or after-hours support as assigned Patient caseload is structured to allow for thorough, patient-centered visits, with time to build relationships and provide meaningful education and emotional support. You will typically care for adult and elderly patients with diagnoses such as cancer, advanced cardiac or pulmonary disease, neurologic conditions, and other complex illnesses. Shifts generally follow daytime hours on weekdays, with the potential for rotating on-call, evening, or weekend coverage depending on agency needs. The role offers a high level of autonomy while still providing a collaborative, supportive environment through regular team communication and case discussions. This position is well suited for RNs who value one-on-one patient time, meaningful end-of-life work, and the opportunity to practice in a community that offers both professional fulfillment and a high quality of life.
