In this role, you will provide skilled nursing care to adult and geriatric patients in their homes throughout Portsmouth and surrounding communities. You will conduct comprehensive assessments, develop and update individualized care plans, and coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team. A typical day may include wound care, medication management, monitoring chronic conditions, patient and family education, and safety evaluations, all delivered in the patient’s home environment.
You will manage a field-based schedule that includes home visits with appropriate time allotted for documentation, communication, and care coordination. Patient acuity and caseload are managed to support safe, high-quality care and allow for meaningful one-on-one interactions. Shifts generally follow daytime hours with some flexibility depending on patient needs and agency scheduling practices.
