Registered Nurse, Case Manager Experience: 5 years RN-MA Certifications: BLS Weekends: None References: 1 Reference in entire work history Supports HUHS patients and their families in coordinating medical, social, emotional, insurance, and vocational services related to illness or injury. Conducts psychosocial assessments in person or by telephone and develops comprehensive outpatient care plans to support safe, effective, and timely care. • Knowledge of discharge planning and care coordination preferred. • Strong communication, collaboration, organization, and clinical assessment skills.
