StartDate: 5/4/2026 Pay Rate: $1600.00 - $2400.00
POSITION SUMMARY – LMSW Social Worker
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization
MINIMUM REQUIRED QUALIFICATIONS – LMSW; BLS; 1 year social work experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8:00AM to 4:30PM with rotating weekends
SYSTEMS – Meditech; CarePort Care Management
START DATE – 5/4/2026
Facility Location
Also known as “Gateway to the Future”, Webster is the cool town to take the kids and have some fun. There are nearby parks and the Space Center in Houston which provides many restaurants and also rides which even adults would enjoy. Snorkeling is close-by and there is even a Gulf Greyhound Park where you can bet on which Greyhound dog will be the winner!
