POSITION DUTIES 
Discharge Planning

MINIMUM REQUIRED QUALIFICATIONS 
Massachusetts (MA) RN Licensure 2 years minimum of acute care hospital experience
Must have at least 6-months experience in large organizations/teaching hospitals with 500 beds or more

LENGTH OF ASSIGNMENT 13 Weeks

SHIFT / HOURS PER WEEK 
Day shift, Monday – Friday (40hrs/week)
