PRE-SCREEN REQUIRED LICENSE VERIFICATION IS REQUIRED AT TIME OF SUBMITTAL dob AND SOCIAL NEEDED Please make sure your candidate can take up to 2 patients at once. Will see 12-14 patients/day Bill Rate: $ Bill rate is set vms fee 6.5% VMS MSP OID: hwl Fees: N/A Modality: Credentialing req: Due 1 week before start 3 references are mandatory Requirements: 2-3 years of General Therapy Experience OP Ortho Experience a plus License: must be active Equipment: Shift/Hours: days Bill GWW:36 Pay GWW:36 OT Bill: On Call: n/a Billing Cycle: WEEKLY Notice:30 Parking: Dress Code: 3 references from within the past year required for candidate to start, no exceptions.
