JOB DESCRIPTION:

• 57 ED (Emergency Department) Beds
• Closed ICU
• Required Procedures - Intubation, Central Lines, ACLS
• EMR (Electronic Medical Record) Charting/dictation - EPIC
• Trauma Designation - Level 3 (New York)
• Subspecialty Backup - Trauma/ Neurosurgery, Cardio, GI, PEDs

SUBMISSION REQUIREMENTS

• NY License
• DEA
• ATLS, ACLS, PALS, Covid Vaccine, Flu, TB Record, Physical and Titers

AS A REMINDER:

• All candidates must have met the minimum shift availability listed on the job.
• CV (Curriculum Vitae) must have all dates in MM/YY to MM/YY format
• Work History must be in Sequential order with most resent-on top
• Any Gaps over 30 days must be explained on the CV
• EPA (Envision Pre-Application) (Please use the attached EPA. It is also in Trio under your Name & then click on Help)
• All Licenses, Boards and Certs with expiration dates must be listed on CV
• Anyone with malpractice must provide a NPDB
• Anyone with legal issues must provide supporting documents

Availability

• 1st date would they be available?
• How many shifts would they be able to do per month?
• What month is their schedule completely open?
• Any shift preferences (Day, Night, Blocksetc.)?
• Any dates in the next 3-6 months they cannot work?
• Any Holidays in the next 3-6 months would they be open to work.?

• 7am-3pm
• 11am-9pm
• 3pm-11pm
• 9pm-7am
• 12pm-12am - PEDs
