Responsibilities:
- Conduct assessments and evaluations of patients to determine their social and emotional needs
- Develop and implement treatment plans based on patient needs and goals
- Provide counseling and support to patients and their families
- Collaborate with healthcare professionals to coordinate patient care
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and ensure their needs are met
- Document patient progress and maintain accurate records
Skills:
- Strong knowledge of special needs populations, including PTSD care, senior care, and nursing home settings
- Proficiency in research methodologies and evidence-based practices
- Familiarity with coding systems such as ICD-10 and ICD-9 for accurate documentation and billing purposes
- Experience in providing cognitive behavioral therapy and motivational interviewing techniques
- Excellent communication skills, both written and verbal
- Ability to work effectively as part of a multidisciplinary team
