What You’ll Actually Be Doing
You’ll manage a dynamic caseload of older adults navigating serious medical, emotional, and social challenges — and you’ll be the steady force helping them stay independent.
You will:
- Conduct in-depth psychosocial assessments and build individualized care plans that make sense in the real world
- Identify risk before it becomes crisis
- Provide individual counseling and facilitate family conferences that bring clarity (and sometimes peace)
- Advocate fiercely — coordinating services, making referrals, and following through
- Lead caregiver groups and workshops that empower families
- Handle discharge planning with professionalism and compassion
- Document like a pro and keep everything airtight
Who You Are:
- MSW degree & an active MSW license in California
- MOCA Certification
- Experienced with geriatric & elderly populations (1 year)
- Proficiency in using Epic (1 year)
- Organized, analytical, and unshaken by shifting priorities
- Confident using clinical judgment independently
- A communicator who can translate complex issues into clear action
- Calm under pressure (even in fast-paced, occasionally chaotic environments)
- Have post-MSW and case management experience
- Have worked in healthcare settings
- Know your way around an electronic health record system
- Are bilingual in Chinese (Cantonese or Mandarin)
