Position Summary
We are seeking a clinically strong and relationship-driven Admissions Nurse to support our growing team. This position is heavily focused on the clinical review of referrals and ensuring appropriate admissions into our facility. The Admissions Nurse works closely with the admissions team and central intake, serving as a key liaison between hospitals, families, and internal departments to facilitate smooth and timely transitions of care.
This role also supports outreach efforts, including regular hospital presence and marketing initiatives, particularly for our specialized ventilator unit.
Key Responsibilities
- Conduct thorough clinical reviews of incoming referrals to determine appropriateness for admission
- Collaborate closely with the Admissions Team and maintain consistent communication with Central Intake
- Perform on-site or remote pre-admission assessments as needed
- Maintain a daily and/or weekly presence in designated hospitals to strengthen referral relationships
- Serve as a clinical resource to hospital case managers and discharge planners
- Support state- and nationwide marketing outreach efforts for our ventilator unit
- Coordinate all clinical aspects of the admission process to ensure a smooth resident transition
- Review hospital records, physician orders, and referral documentation for accuracy and compliance
- Initiate care plans upon admission and communicate resident needs to the nursing team
- Verify payer source information and collaborate with billing as needed
- Ensure all documentation meets state and federal regulatory standards
- Support facility census development goals
Qualifications
- Current, active RN license in [State] (LPN considered based on experience)
- Clinical experience in skilled nursing, long-term care, ICU, respiratory care, or hospital setting preferred
- Experience with ventilator or high-acuity patients strongly preferred
- Strong clinical assessment and critical thinking skills
- Knowledge of Medicare, Medicaid, and managed care processes preferred
- Excellent communication, organizational, and relationship-building skills
- Ability to work independently while collaborating across departments
- Proficiency with EMR systems
