JOB REQUIREMENTS: As a Registered Nurse (RN) - Case Manager in Amery
Hospital & Clinic\'s Med/Surg unit, you are responsible for coordinating
the care/service of selected patients while developing professional
relationships with medical staff, clinical team, community providers and
payers to promote effective coordination of care across the continuum of
services. You will also monitor the appropriateness of hospital
admissions and extended hospital stays Work Schedule: .5 FTE / 20 hours
weekly. Required Qualifications: Graduate of an accredited school of
nursing Possess a current WI RN license. BLS certification required and
ACLS certification must be obtained within 6 months of hire. Preferred
Qualifications: Hospital nursing experience. Case Management experience.
Flexible, organized and functions well in stressful situations.
