- Co Develops, implements and monitors the clinical management of parenteral and enteral patients as part of the Nutrition Support Team
- Makes recommendations for nutrition intervention when necessary
- Reviews patient medical history, anthropometrics and pertinent lab values to assess home nutrition support protocols and insurance payer requirements
- Counsel patients and caregivers on home enteral and parenteral nutrition therapy. Assesses patients’ understanding of and compliance with prescribed diet principles
- Ensure continuity of care with patient’s physician, nurses and nutrition support teams to provide high quality patient centered care
- Participate in Performance Improvement Program and ACHC accreditation
- Identifies and promotes nutrition program services to clinical and professional referral base and performs necessary in-service education
- Supervisory Responsibility: No
- Bachelor of Science in Nutrition or related field
- Registered Dietitian with license to practice in assigned state(s); Certified Nutrition Support Clinician preferred
- Minimum of two (2) years of clinical experience required.
- Must have strong communication skills and organizational skills
- High level of self-motivation, initiative, flexibility and ability to work under pressure
