2025-2026 Perry Family Free Clinic Associate
Interested in a healthcare profession(Medical school, Dental school, Public Health, Master's Program)? Looking to serve your community, grow your leadership abilities and gain management experience? The Perry Family Free Clinic exists to improve access to care for Black men in Dane County by providing professional primary care and disease management. The clinics vision is to Disrupt unhealthy life cycles for Black men by establishing trust and improving their health and the quality of life.
Member Duties : • Community Engagement - Meeting with Dane County Social Workers to help identify community residents lacking healthcare and in need of a medical home. • Supporting the Perry Family Free Clinic (PFFC) Physicians by doing patient check-ins on clinic days. • Supporting UW Madison School of Pharmacy Students - Operation Diabetes program. • Helping PFFC patients complete Health History Questionnaires. • Phone call follow-up with patient appointment reminders. • Occasionally picking up and dropping off PFFC patients with agency vehicles.
