The Physical Therapist (PT) is responsible for developing and implementing a physical therapy program to treat the patient’s medical diagnosis(s) and/or for functional maintenance restoration affected by the patient’s illness or injury. The PT will develop and update, as often as indicated, an individualized plan of care including, but not limited to the patient’s functional levels, goals, interventions, and rehabilitation potential.

Education
- Graduate of an approved school of Physical Therapy (PT).
- Current state Physical Therapy license.
- Must meet initial and ongoing educational requirements as stipulated by the state to maintain an active professional license.
- Current CPR card, valid driver’s license, reliable transportation, automobile insurance.
