Job Summary:
The QAPI practitioner manages the organizations Quality assurance and the Quality Reporting Program. They assist with Electronic Medical Record (EMR) Documentation, Clinician Education, Team Support and Outcome Performance as well as department based specialty projects and responsibilities. Position includes driving for field evaluations and home visits.
Essential Responsibilities:
• Instruct staff on their responsibility in following the Compliance and Ethics Program
• Stays abreast of regulatory changes and with collaboration from their supervisor, implements required changes within the organization.
• Detect non-compliance and follow up as appropriate.
• Provide organization support for all QAPI initiatives including but not limited to data collection, trending and improvement initiatives. Will lead and facilitate improvement activities as needed in keeping with the QAPI Plan such as Performance Improvement Projects.
• Support the Problem Identification System Analysis (PISA) process including documentation and site visits as required.
• Act as a resource and trainer for the quality analytics programs and customer satisfaction programs selected by the organization.
• Initiate customer service driven improvement projects/education based on current trends and CAHPS, HHCAHPS results.
• Provide clinical team support for QAPI and Compliance initiatives geared to improving outcomes, customer satisfaction, and successful external review.
• Complete field evaluations and home visits to ensure quality of care and/or initiate and recommend improvements.
• Oversee adverse event reporting for assigned areas of responsibility.
• Provide IDG support including documentation review, feedback and follow up, including as it relates to trending/site visit education and medical necessity outlier cases.
• Serve as an active coach for clinical and non-clinical teams to ensure increased understanding of Hospice/Home Health criteria and documentation requirements. This is accomplished through one on one coaching, EMR communication and ongoing support and education.
• Support all aspects of all Avow quality/regulatory education initiatives from planning, assessment, program development and hands on training and support.
• Assist with quality reporting data processing and reporting for assigned teams in keeping with regulatory requirements.
• Initiate and recommend policy and procedure changes for the agency in keeping with regulations and best practices.
• Assume direct responsibility for assigned areas within the Joint Commission standards or CHAP standards and ensure compliance through assessment, education, monitoring and policy development.
• Perform internal audits as assigned as part of the annual corporate compliance plan and QAPI plan, assist with determining current priorities to ensure organization compliance.
• Analyze patient records to ensure compliance with State, Federal, Joint Commission and CAHP regulations and standards.
• Serve as a role model for customer satisfaction and investigates customer concerns at the request of the Compliance Officer ensuring timely resolution.
• Create monthly/quarterly reports summarizing quality/compliance activity and trends for use for not only for the Department but Committees as needed.
• Provide on-site survey support including preparation and post survey activities.
• Participate actively in organization committees as assigned.
• Other duties as assigned.

Qualifications:Education/Experience:
An Associate's or Bachelor’s in nursing degree with a current LPN or RN Florida license; Master’s degree desired. A minimum of 5 - 8 years’ experience in a healthcare organization including exposure to Homecare, to include demonstrated leadership, familiarity with operational, financial, quality assurance, and human resource procedures and regulations; The Joint Commission Standards of care; OIG Corporate Compliance Program; Federal and State regulatory requirements and compliance guidance for health care providers.
Certificates, Licenses, Registrations:
Licensed Practical Nurse or Registered Nurse licensed in the state of Florida.
Valid Florida driver’s license.
Preferred certification in Healthcare Quality (CPHQ), and/or Certified Hospice and Palliative Nurse CHPN®. CHPN Certification to be completed within1 year from position start date (Hospice).
CPR certification required.
Supervisory Responsibilities:
There are no direct reports for this position.
