Job Summary:
The Registered Occupational Therapist will provide diagnostic evaluations and treatment to assigned patients, and help patients reach maximum performance level and to use skills learned to the fullest to function in the community within the limits of their capabilities.
Job Duties:
- Provide OT services prescribed by the physician which can be safely provided in the home to include: Initial and ongoing assessment to determine level of functioning, including OASIS assessments at appropriate time point (G333, G336).
- Developing and revising the plan of care in consultation with the physician and other care team members (G162, G186).
- Providing therapeutic treatments and evaluations of equipment needs as applicable.
- Advising and consulting with the family and other agency personnel as applicable regarding medequip, HEP, Safety Issues (G143, G 188).
- Prepare clinical and progress notes (G187).
- Evaluating outcomes of care.
- Supervising Home Health Aides as appropriate (G190).
- Planning for discharge.
- Conduct themselves in a professional manner whenever representing the company.
- Will maintain HIPAA Compliance.
- Participate in in-services and quality assurance activities.
- All other duties as assigned.

Qualifications:Education/Experience:
Bachelor’s degree in occupational therapy from an accredited OT program required. One year of home health experience required
Certificates, Licenses, Registrations:
Valid Florida Driver’s License
Florida Occupational Therapist License
Valid CPR Certification
Computer Skills:
Knowledge of basic typing skills, data entry and word processing software, database software and general knowledge of e-mail.
Language Skills:
Ability to read, analyze, and interpret common scientific and technical journals. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to top management, public groups, and/or boards of directors.
Mathematical Skills:
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Reasoning Ability:
Ability to define problems, collect data, establish facts, and draw valid conclusions.
