CNA (PRN) -HOME HEALTH CARE

*This is a PRN position. Florida Certified Nursing Assistant License preferred. 

Job Summary:

The Home Health Aide (HHA) provides personal care and related support services in the home setting under the supervision of a Registered Nurse (RN). The HHA performs assigned tasks as outlined in the client's individualized plan of care and under no circumstances shall perform any intravenous, sterile, or invasive procedures, with the exception of taking rectal temperatures.

Responsibilities:

• Follow the established plan of care to provide safe and competent care
• Assist clients with personal hygiene and grooming
• Maintain a clean, safe, and healthy living environment
• Encourage client independence in accordance with the care plan
• Support clients in mental and emotional well-being through engagement in meaningful activities
• Assist with light housekeeping duties as assigned.
• Assist with ambulation, prescribed exercises, and range of motion (ROM) as assigned
• Provide emotional support and foster trusting relationships with clients and families
• Promptly report any changes in client condition or needs to the supervising nurse
• Complete and submit timely visit reports and documentation
• Coordinate weekly schedules and services with other care team members
• Attend all required in-service training and continuing education
• Communicate regularly with supervising nurse regarding client status and care planning
• Perform duties within the HHA scope of practice and under direct RN supervision

Education/Experience:
Must be 18 years of age or older. High school diploma, or general education degree (GED) preferred. Florida Certified Nursing Assistant (CNA) License preferred; or completion of at least 40 hours of approved Home Health Aide training or successful completion of a Home Health Aide competency exam as required by state regulations.

Certificates, Licenses, Registrations:
Certified Nursing Assistant (CNA) meeting the requirements of State of Florida and Medicare regulations preferred; or Home Heath Aide Certification
Valid Florida Driver's License.
Valid Automobile Insurance.
Valid Basic Life Support (BLS)

Computer Skills:
Strong computer skills including database software and general knowledge of e-mail, educational, and other technology programs outlined by the organization necessary for the role.

Language Skills:
Communication skills of reading, writing, and speaking in English. Bilingual (fluent in Spanish) a plus.

Math Skills:
Math skills including addition, subtraction, multiplication, division, and percentages/fractions.

Field evaluations involve driving to patient homes.
