American Traveler is hiring a travel RN for a Hospice & Palliative Care Home Health position in New Jersey, requiring a minimum of 2 years of home hospice nursing experience and a valid NJ driver's license.
Details- Home health / hospice setting with a focus on palliative and end-of-life care
- Day shift schedule, 8:00 AM – 4:00 PM, 5 days per week at 7.5 hours per day (37.5 hours/week)
- One weekend per month required
- Floating to different counties as needed
- EMR: Epic (experience required)
- 13-week contract
- Active NJ RN license (or compact license with NJ privilege) required
- Current BLS certification required
- Minimum 2 years of RN experience in a home hospice environment required
- Valid New Jersey driver's license required
- ASN or RN Diploma with active NJ RN license required
- Proficiency with EMR systems required; Epic experience strongly preferred
- Strong critical thinking and communication skills required
- Two professional references required for consideration
- Conducts comprehensive patient assessments to determine appropriateness for hospice care and develops individualized care plans for patients and families
- Manages a caseload as the primary case manager, including coordinating interdisciplinary team (IDT) communication and attending IDT meetings every two weeks
- Performs laptop data transfers twice daily and completes admission and re-visit documentation within required timeframes
- Responsible for medication reconciliation, pharmacology knowledge, and safe medication management
- Conducts home health aide (HHA) supervisions per established standards
- Participates in performance improvement activities, utilization review meetings, and provides mentorship and orientation support to new staff
- Candidates must reside more than 50 miles from the facility — local candidates are not eligible
- Travelers may float to a nearby sister facility as needed
