Additional Information
- Responsibilities include patient evaluations, individualized treatment planning, and ongoing reassessment
- Manages discharge planning from first visit through treatment completion
- Supervises and delegates to ancillary staff and students per state law and department policy
- Maintains legible, accurate, and timely documentation in accordance with accreditation and insurance requirements
- Communicates with patients, families, and care teams using AIDET principles
- Provides patient and family education based on identified needs
- Participates in professional development and quality improvement activities
- Maintains a safe environment for patients, visitors, and staff
- Candidates employed by the facility within the past 12 months may not be eligible for submission
