Description
American Traveler is seeking a Physical Therapist with Home Health experience for a 13-week travel contract serving a geriatric patient population across a defined territory in the East Bay area of California.
Details
- Home health setting serving a geriatric patient population
- Territory covers the Far East region, approximately from Discovery Bay to Pittsburg, within a 60-mile radius of the hub/office
- Average caseload of 5–7 patients per day
- Day shift schedule, 5x8-hour shifts (8:00–17:00), Monday–Friday
- No on-call or weekend requirements
- EMR: HCHB (experience required); OASIS experience preferred
- iPad provided for documentation; personal cell phone not required
- Mileage reimbursed at the IRS standard rate
Requirements
- Current PT license required if applicable
- Minimum 1 year of recent home health physical therapy experience
- Current BLS certification required
- Proficiency with HCHB EMR required
- OASIS experience preferred
- Must be flexible with territory coverage
- 2 professional supervisor references from within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
