- Valid U.S. Social Security Number required
Additional Information
- No employment gaps of 90 days within the past 12 months
- Up to 5 days of requested time off allowed, must be made up during the assignment
- Local candidates accepted, though a radius rule may apply for pay rate and contract eligibility
- Candidates living within 50 miles of the facility are limited to 3 consecutive 13-week contracts before requiring a 1-year separation or transfer to a facility outside the radius
