Qualifications:
- Active Licensed Vocational Nurse (LVN) license
- 1-2 years of hospice or home health experience preferred
- Proficiency in EMR systems, Microsoft Office (Excel/Word)
- Strong data entry and reporting skills
- Knowledge of CHAP and CMS standards
- Effective communication and teamwork abilities
Responsibilities:
- Collaborate with the QAPI Coordinator to develop and implement QAPI initiatives
- Conduct pre-billing audits, targeted chart reviews, and data collection/validation for the HOPE Tool
- Ensure accuracy, regulatory compliance, and alignment with CHAP and CMS standards
- Support high-quality patient care through compliance efforts
- Assist in internal and external audits as required
- Stay informed of regulatory changes and best practices in QAPI
