- Must have previous experience as a Social Worker
- Must have long-term care experience/ nursing home experience
- Must be a self-starter and energetic
- Strong written and verbal communication skills
Social Worker Job Description:
- Understand and meet all government requirements for social service documentation.
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living.
- Create personalized care plans that address both immediate and long-term needs.
- Provide information to the resident, and/or family member, on residents' rights and responsibilities.
- Provide counseling and emotional support to help residents and their families navigate the challenges of long-term care.
- Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents.
- Participate in quality assurance interdisciplinary team meetings.
