The Opportunity: This isn't your typical SNF role. You'll be at the forefront of our acute intervention program, working with clinical autonomy and physician support to manage complex patients right where they live. By combining in-person rounds with strategic telehealth coverage, you'll stabilize residents during critical moments and prevent the trauma and disruption of unnecessary hospital transfers.
Key Responsibilities: ·
- Conduct psychiatric evaluations and diagnostic assessments·
- Develop and manage individualized treatment plans·
- Prescribe and monitor psychiatric medications·
- Provide psychotherapy and patient education when appropriate·
- Collaborate with therapists, primary care providers, and other healthcare professionals·
