We are currently seeking a Licensed Practical Nurse (LPN) to join our clinical team. This role is responsible for providing direct nursing care under the supervision of the RN, administering medications and treatments as ordered, supporting care plan implementation, and ensuring residents receive safe, high-quality care in accordance with clinical and regulatory standards.

Job Responsibilities and Duties:
Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
Provide wound care: follow protocols, irrigate, pack, and remove dressings
Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
Respond effectively to emergency situations
